“LLMs are being rapidly adopted without forethought for repercussion. For instance, UK judges are allowed to use LLMs to summarise court cases and, on the medical side, public medical question answering services are being rolled out. Our vision addresses the socio-technical limitations of LLMs that challenge their responsible and trustworthy use, particularly in medical and legal use cases.”
Large Language Models (LLMs), like those used in ChatGPT and virtual assistants, are cutting-edge artificial intelligence algorithms trained on massive amounts of text data. They can generate human-like text as well as creative content, translate across languages, and answer questions in an informative way. However they have known technical limitations such as biases, privacy leaks, poor reasoning and lack of explainability, which raises concerns about their use in critical domains such as healthcare and law. Our vision is to address socio-technical limitations of LLMs that pose a challenge for their responsible and trustworthy use, considering use cases in legal and medical contexts. This includes two core strands:
Background & challenges: Regulatory frameworks, guidance and proposed legislation have been introduced to address privacy, accuracy and explainability of automated systems (EU AI Act, UK Government’s National AI Strategy, UK Algorithmic Transparency, UK Government National Data Strategy, UK White Paper: A Pro-innovation Approach to AI Regulation, UK ICO guidance on explaining AI). It is not clear how domain experts can translate this regulation to meet needs for LLM technology use.
Foci & implementation: We will review and synthesise regulatory expectations and principles of ethical and responsible innovation into a set of requirement criteria for safe and responsible use of AI within medicine and the law. Such criteria are expected to include e.g. privacy preservation, inclusivity, factuality, situational awareness, explicability.
Background & challenges: State-of-the-art evaluation of LLMs has resulted in large benchmarks containing hundreds of linguistic and programmatic tasks (e.g. reasoning, mathematics, context free QA) and associated metrics meant to assess LLM capabilities. While such benchmarks offer a more challenging evaluation setting for AI systems, requiring more than memorisation ability for many of the tasks, they tell us nothing about the real-world applicability of LLMs in social applications. Evaluation of GPT-4 on medical QA has shown impressive results, with GPT-4 performing 20 points above average on samples from the U.S. Medical Licensing Examination. However, solving exam multiple-choice questions is far removed from the ability to diagnose or monitor a patient over time.
Foci & implementation: We will operationalise satisfiability of the RRI criteria identified in Work Stream 1 by implementing novel metrics and tasks developed through co-creation. Where appropriate strategies (e.g. memorisation prompting), metrics, tasks and data from existing benchmarks will be included (e.g. counterfactual tasks, inference-based metrics from big-bench or HELM) with the addition of new metrics (e.g. for privacy, inclusivity) and a focus on medical and legal needs.
Background & challenges: Most social applications, be it summarisation of long documents (e.g. court cases, therapy sessions) or dialogue, require contextual awareness, both in terms of temporal and situational reasoning, shown to be lacking in LLMs. Specifically LLMs lag behind human performance and even small-scale specialised models on tasks such as temporal ordering of timelines, satisfaction of temporal constraints (before and after) and common sense knowledge about events.
Foci & implementation: To mitigate shortcomings in temporal and situational awareness we will investigate instruction fine-tuning for temporal relations and use-case relevant situations informed by domain experts and responsible AI principles.
Background & challenges: Dialogue systems are the leading public facing AI technology. Yet their suitability for use with sensitive data or vulnerable individuals is questionable. Issues include privacy leaks, harmful biases, inappropriate responses, poor situational reasoning and hallucinations. These constitute limitations that hinder appropriate generation (correct, safe and suitable), relevant to both summarisation and dialogue needs within use cases.
Background & challenges: LLM use in medicine raises concerns and challenges, reflective of the socio-technical limitations already discussed in use cases 1 and 2, and Work Streams 2 and 4. These are particularly prominent in medical diagnostics and monitoring because of the complexity of data sources and the potential impact associated with wrong or missed diagnoses and critical points.
Our current use cases and partners are in: (1) Multi-modal medical diagnostics and monitoring for cancer and mental health; (2) AI support for mental health covering summarisation & dialogue self-management interventions; (3) AI legal support covering long-form summarisation and dialogue. We are considering new partners and use cases.
The morning session covered research in creating time sensitive sensors for mental health from Prof. Maria Liakata’s Turing AI Fellowship, including presentations on identifying of moments of change with TempoFormer by Talia Tseriotou, capturing timelines and identifying changes with neural Hawkes processes by Anthony Hills, timeline summarisation and improving temporal reasoning by Jiayu Song, framework and benchmark for reasoning limitations in LLMs by Mahmud Akhter, evaluation of synthetic language data and LLMs by Jenny Chim, and a dashboard for mental health monitoring by Sebastian Lobbers.
The afternoon began with talks by Dr. Becky Inkster, who presented on multi-modal data for mental health citizen science, and Prof. Dana Slonim, who presented about the MIND framework, the CLPsych 2025 shared task, and work at the intersection of AI and psychotherapy in her lab. Next, Dr. Onno Kampman gave an overview about AI applications implemented by the MOH Office for Healthcare Transformation (MOHT) in Singapore. Dr. Jen Martin from MindTech gave a talk about the potentials, priorities, and barriers to using AI to address unmet needs in mental health. Finally, from the AdSoLve project team, Prof. Domenico Giacco from the University of Warwick presented about the AI-DIALOG+ system.
The talks were followed by an introduction to the project’s evaluation platform and structured discussions around requirement identification on two use cases: (1) Summarisation for therapy sessions, and (2) Dialogue for self-management.
Our launch event took place on December 5th 2024. We started with four talks from the project team. Prof Maria Liakata gave an overview of the project, presenting goals and progress so far in terms of publications and next steps. Then, Prof Greg Slabaugh gave a talk about the first use case of the project, in multi-modal medical diagnostics and monitoring, focussing particularly on the goals and technical challenges involving multi-modal cancer research diagnostics. Next Prof Domenico Giacco gave a talk about another major use case of the project, on AI support for mental health, focussing on requirements and challenges for summarisation, monitoring and dialogue self-management. Finally Dr Jiahong Chen presented our third use case, on AI legal support, highlighting our findings from our recent literature review on applications of LLMs in the legal domain. All three use case presentations ended with a diagram showing the potential translation of requirements for different tasks based on LLM systems into evaluation criteria, metrics and subtasks to evaluate system performance and appropriateness.
Following the presentations by the project team in the morning, we had talks from project partners. Jonathan Pearson from NHS England talked about thoughts on evaluation of LLM-based applications in the context of the NHS. Hannah Richardson from Microsoft Futures talked about steps towards understanding risk management for generative AI as a medical device. Zachary Goldbeg from Trilateral Research discussed how responsible innovation had been incorporated in pre-LLM solutions that could inform current approaches. Henry Sturm from LegalGeek talked about the state of legal tech in the UK and the role of AI.
In the afternoon we had a presentation of an evaluation platform for assessing tasks performed by LLM systems and a themed discussion on this topic, inspired by the diagrams of requirements, tasks and metrics presented in the use case talks in the morning. Following the presentation we split into three groups per use case to discuss requirements, challenges and evaluation needs for each use case.
